We Learned the Hard Way

Healthcare Is Complex & Fragmented

Founded in 1999 as an EMR, RiskAssist MD emerged from within the realm of urology clinics that were into patient navigation and drowning in administrative overload — missed follow-ups, untracked lab results, and rising liability risks.

After seeing patients fall through the cracks, we realized the issue wasn’t lack of care — it was lack of process visibility. Traditional reminder systems and EMRs were never built to manage the complexity of ongoing clinical navigation.

Clinics needed something smarter, adaptive, and built around how care actually happens — not how software thinks it should.

Reinventing Clinical Follow-Up

Conventional EMRs record events. They don’t guide action. The physicians, nurses, and staff of clinics were tired of managing reminders through spreadsheets and sticky notes.

So we reimagined what follow-up could look like — a rule-based workflow engine that learns from clinic operations and automates patient engagement:

1999

Where It All Began — The EMR Era

RiskAssist started as an electronic medical record (EMR) company in 1999.
 At the time, EMRs were revolutionary — replacing paper charts, helping clinics bill, schedule, and document patient visits.

But as the system evolved, one flaw became clear:

EMRs could record data — but not see patterns or prevent risks.

They lacked the ability to mine, connect, and act on critical patient information. Data sat in silos — and patients were falling through the cracks.

2010–2015

The Rise of Lawsuits & Weak Clinical Processes

By the early 2010s, lawsuits in healthcare were becoming more frequent and more severe.

 Clinics were often blindsided — not because they didn’t care, but because their systems didn’t alert them when something went wrong.

Manual processes, inconsistent reminders, and fragmented data led to serious misses:

  • Patients not notified of positive test results
  • No-shows and missed recalls slipping through the cracks
  • Lack of documentation proving communication
  • One missed follow-up could now cost a clinic $150K+ in legal fees.

RiskAssist began positioning itself as a protective layer over EMRs — to catch the missing pieces before they became legal or patient-safety issues.

2016–2019

Filling the Gaps EMRs Couldn’t

EMRs were built for compliance, not intelligence. They didn’t have automated triggers, risk alerts, or patient buckets for unresolved cases.
RiskAssist changed that.

 We introduced a data-mining and trigger engine that identified high-risk patients who had unaddressed results or incomplete follow-ups — for example, a patient diagnosed with prostate cancer whose positive result was known to the clinic but never communicated to the patient.

Our system created digital “risk buckets” — identifying patients who were diagnosed, overdue, or uncontacted — giving clinics a structured process to close every loop.

2020

A New Mission: From Risk Management to Navigation

In 2020, we redefined our mission.

 What started as a risk reduction platform became (grew into?) a patient navigation system — focused not only on protecting clinics, but on guiding patients through care pathways.

RiskAssist began automating:

  • Follow-up tracking from test → result → communication → treatment
  • Structured messaging with read receipts
  • Process flows customized by clinic rules
  • Recall letters and rebooking for canceled slots


Our goal evolved to help clinics prevent risks before they occur — not just respond when they do.

2021–2023

Adoption & Resistance

Clinic adoption was our biggest challenge.

Many practices were still fatigued by years of EMR change.

CMS mandates had already forced them to adopt certified EMRs, and providers were wary of new technology investments.

 Private equity acquisitions only deepened the tension — physicians wanted flexibility, while boards demanded standardization.

RiskAssist positioned itself as the “last mile” solution — integrating with existing EMRs rather than replacing them, reducing complexity, and showing tangible ROI through recovered revenue and reduced liability.

2024–2025

Closing the Loop on Patient Care

Today, RiskAssist MD has evolved into a comprehensive AI-enabled patient engagement and compliance platform.

 We automate clinical reminders, track risk exposure, and ensure patients are contacted — every time.

Our focus remains simple:
To fill the gap EMRs never could — the last mile between clinical data and patient action.
